AHPC Scope of Practice for MLTs – Labex Blog

Introduction: A Landmark Step for MLTs in Pakistan

The Allied Health Professionals Council (AHPC) has officially approved and published the Scope of Practice for Medical Laboratory Technology (MLT) professionals in Pakistan. This is a historic development that defines, for the first time in a structured manner, exactly what each level of laboratory professional is authorized to do — and what they are not.

Whether you are a fresh diploma holder working as a Lab Technician or an experienced PhD-qualified scientist running a diagnostic facility, this document draws a clear boundary around your professional role. Understanding it is not optional — it is binding and enforceable under national regulatory standards.

This article breaks down the AHPC document in plain, easy-to-understand language so that every MLT professional can know their rights, responsibilities, and limitations.

Why Does This Document Matter?

Before this document, there was widespread confusion regarding who can do what in laboratory settings. Lab assistants were sometimes performing tests beyond their qualification, while highly qualified scientists were not being given the authority they deserved. This scope of practice fixes all of that.

Here is what it ensures:

  • Standardized, ethical, and competency-based laboratory services
  • Clarity on reporting authority — who can sign and issue reports
  • Supervisory hierarchy — who supervises whom
  • Eligibility for independent laboratory operations
  • Compliance with national regulatory and accreditation standards
⚠️ AHPC Registration is MANDATORY for All Categories Without AHPC registration, you cannot legally practice in your designated role — regardless of your qualification level.

4. Medical Laboratory Technologist (MLT)

Eligibility
BS MLT (16 years of education) or BSc MLT (14 years) with/without MSc in relevant field, from AHPC-recognized institute. HEC equivalence mandatory if required.

This is the core professional category of the MLT field with broad authority including reporting, laboratory establishment, and teaching.

What they CAN do:

  • Assist laboratory management; act as Assistant Lab Manager / QC Manager / HR Manager
  • Perform all specialized analyses
  • Perform QC and QA for total quality management
  • Manage accreditation activities (ISO / CAP / JCIA / Health Care)
  • Issue reports WITHOUT clinical assessment
  • Arrange advanced technical training in the lab
  • Can establish a Basic Lab Testing Facility, Independent Blood Bank, and Collection Point (except Cytopathology, Immunohistochemistry, Histo-pathological, and Bone Marrow reports — MPhil required for these)
  • Can establish a highly equipped lab with all routine and special facilities (PhD in relevant MLT field required)
  • Teach undergraduate students; assist in curriculum development
  • Conduct research and publish in peer-reviewed journals
  • Can conduct ISO-9001 and ISO-15189 Audits after relevant certification

5. Specialised Medical Laboratory Technologist

Eligibility
BS/BSc MLT + MSc in relevant field + 5 years experience AND MS/MPhil in relevant MLT field. AHPC registration mandatory.

  • Teach undergraduate and postgraduate students; supervise thesis
  • Apply for research grants and guide junior faculty research

6. Medical Laboratory Scientist

Eligibility
BS/BSc MLT + MSc (basic qualification) AND PhD in relevant MLT field (higher qualification). AHPC registration mandatory.

The Medical Laboratory Scientist is the highest category, carrying full authority for laboratory operations, research leadership, and academic governance.

What they CAN do:

  • Act as Head of Department (Technical Domain)

🔬 Laboratory Registration: Who Can Open a Lab?

This is the most practically important section for any MLT professional considering independent practice. The AHPC document specifies two categories of private laboratory operations:

i. Stat Labs / Collection Points / Blood Banks

Requirement
Registered Medical Lab Technologist with 5 years of experience AND MPhil/MS degree in relevant MLT field. Must possess a basic degree in Medical Laboratory Technology.
Note: A standard BS MLT alone is NOT sufficient to independently run a stat lab. You need both postgraduate qualification AND five years of experience.

ii. Full-Range Clinical Laboratories

Requirement
Operable by PhD holders in relevant fields. Must possess a basic degree in Medical Laboratory Technology. Specialized procedures (histopathology, bone marrow) must be conducted in the presence of a qualified Hematologist / Histopathologist.

Summary: Lab Registration at a Glance

Lab Type Minimum Qualification Additional Condition
Stat Lab / Collection Point / Blood Bank BS MLT + MPhil/MS + 5 years experience AHPC Registration
Full-Range Clinical Laboratory BS MLT + PhD in relevant field Specialized tests need Hematologist / Histopathologist on site

Limitations: What ALL MLPs Cannot Do

Regardless of qualification level, all Medical Laboratory Professionals (MLPs) share these firm and non-negotiable boundaries:

Limitation 1: MLPs do NOT prescribe medications or diagnostic tests. Prescribing is the sole domain of registered medical practitioners.
Limitation 2: MLPs do NOT perform or report histopathological examinations or bone marrow procedures independently. These require a qualified Histopathologist or Hematologist.
Limitation 3: MLPs focus exclusively on laboratory-based analysis and support for clinical decision-making. They are non-clinical healthcare practitioners.

Key Takeaways for Every MLT Professional

1
AHPC registration is mandatory for ALL categories — without exception, regardless of qualification.
2
Your scope is defined by your qualification level — always work within it.
3
Reporting authority increases with qualification: Diploma holders assist; BS/MSc holders report; PhD holders have final authority.
4
To open your own lab, you need postgraduate qualifications — MPhil/MS minimum for stat labs; PhD for full-range labs.
5
The 1-year certificate (Lab Assistant) and 2-year BSc programs are being phased out — no new admissions are authorized.
6
Special tests like histopathology and bone marrow still require specialized physicians — regardless of your qualification.
7
This document is legally binding — violations can result in regulatory action under AHPC.

Conclusion

The AHPC Scope of Practice for MLT is not just a regulatory document — it is a declaration that Medical Laboratory Professionals are recognized, valued, and protected under a national framework. It elevates the profession by creating clear pathways for career growth, defines authority at each level, and protects the public by ensuring only qualified individuals perform specific laboratory functions.

For every MLT professional in Pakistan, this document should be read, understood, and followed. It is your professional roadmap.

If you have not yet registered with AHPC, do so immediately. And if you are planning to open a private laboratory or collection point, make sure you meet the qualification and experience requirements set out in this scope before proceeding.
